How much would it cost for Verizon to unlock a phone?

I have an Intensity2 , and I was playing with the setting and I clicked NAM 2 . and it restarted and locked my phone. I tired 0000 and everything.I also need my phone activated. So how much would it cost to unlock my phone. Please Help.
